TL;DR — Three Attack Surfaces, Four Failure Modes

SIXTY-EIGHTH MECHANISM CLASS: GPRC5D as a Novel Multiple Myeloma Target and the First GPCR-Directed Bispecific T-Cell Engager

The sixty-seventh mechanism class — mosunetuzumab (Lunsumio; first CD20×CD3 bispecific T-cell engager ever; CrossMAb IgG1 LALA-PG; attack #640; blog #145) — established the CD20×CD3 bispecific modality in follicular lymphoma using CrossMAb technology. The SIXTY-EIGHTH CLASS extends the bispecific T-cell engager concept into a completely novel target class: the G-protein coupled receptor (GPCR) superfamily, and specifically the orphan class C GPCR family member GPRC5D.

GPRC5D TARGET DISCOVERY: GPRC5D (G Protein-Coupled Receptor Class C Group 5 Member D; also RAIG4; gene chromosome 12p13.3; MW ∼35 kDa; 7-transmembrane helix topology) was identified as a multiple myeloma surface target through comparative transcriptomics of MM bone marrow plasma cells vs normal donors. GPRC5D mRNA is strongly overexpressed in MM plasma cells relative to normal tissues with a favorable expression pattern: HIGH on MM plasma cells (~90–95% specimens); DETECTABLE on skin keratinocytes, nail matrix cells, and taste bud cells; ABSENT on most other tissues including hematopoietic progenitors, T cells, NK cells, liver, kidney, lung. This expression profile creates a therapeutic window: T-cell cytotoxicity against MM plasma cells >>> normal tissue damage, BUT non-zero GPRC5D expression in skin/nail/taste creates the pharmacological toxicity fingerprint distinguishing talquetamab from all prior myeloma therapeutics.

GPRC5D STRUCTURAL BIOLOGY: GPRC5D belongs to Class C GPCRs (Glutamate receptor family), which includes metabotropic glutamate receptors (mGluR1-8), GABA-B receptor, calcium-sensing receptor (CaSR), and GPRC5A/B/C/D subfamily. Class C GPCRs share a large bilobed N-terminal extracellular Venus Flytrap Domain (VFD) that binds agonists/amino acids. GPRC5D is an ORPHAN CLASS C GPCR: the VFD in GPRC5D is TRUNCATED/VESTIGIAL — shorter N-terminal extracellular domain; the ligand-binding pocket is absent or non-functional; no endogenous amino acid, metabolite, or peptide ligand has been identified. The absence of an endogenous agonist means: (a) small molecule drugs targeting the orthosteric site are not feasible; (b) the receptor cannot be modulated by conventional pharmacology against its natural ligand; ONLY monoclonal antibodies or bispecific T-cell engagers recognizing extracellular loop epitopes of GPRC5D can engage it therapeutically. This is why talquetamab is FIRST-IN-CLASS: prior bispecifics (blinatumomab [CD19×CD3; class 57], teclistamab [BCMA×CD3; class 66], mosunetuzumab [CD20×CD3; class 67]) all target type I transmembrane proteins with known biology; talquetamab is the first to target a 7-TM GPCR, validating the GPCR class as a bispecific antibody target.

GPRC5D vs BCMA: BCMA-INDEPENDENT EXPRESSION is the critical therapeutic implication. GPRC5D expression does NOT correlate with BCMA expression. MM cells that have LOST BCMA (dominant BCMA antigen-escape mechanism after belantamab mafodotin, teclistamab, or BCMA-CAR-T therapy: ~20–30% of patients after BCMA-CAR-T) RETAIN GPRC5D expression at high levels. Talquetamab is therefore viable for post-BCMA-therapy relapse — a population with no prior standard of care. The NORMAL HEMATOPOIETIC STEM CELL SPARING advantage: GPRC5D is absent from HSC and CD34+ hematopoietic progenitors, so talquetamab T-cell cytotoxicity does not ablate hematopoiesis. The ON-TARGET OFF-TUMOR NORMAL TISSUE TOXICITY from skin/nail/taste bud GPRC5D expression is the key limitation producing talquetamab's pharmacological fingerprint.

None of this GPRC5D biology — Class C orphan GPCR classification, vestigial VFD, 7-TM structure, MM plasma cell expression, skin/nail/taste bud normal tissue expression, BCMA-independent targeting rationale, or GPCR-bispecific T-cell engager mechanism — is present in Cority, VelocityEHS, or EHS Insight at CAS 2278847-52-6.

DuoBody IgG4 Technology: Controlled Fab-Arm Exchange, Phe405Leu + Lys409Arg, and SC Hyaluronidase Formulation

DuoBODY PLATFORM OVERVIEW (Genmab A/S; Copenhagen/Utrecht; Labrijn AF et al. Nature Protocols 2014): DuoBody generates full-length IgG bispecific antibodies through CONTROLLED IN VITRO FAB-ARM EXCHANGE (CFE) of IgG4 antibodies. IgG4 NATURAL FAB-ARM EXCHANGE: wild-type IgG4 antibodies undergo spontaneous in vivo Fab-arm exchange at low levels (Labrijn AF et al. Nature Biotechnology 2009) via partial disassembly of “half-molecules” (one heavy chain + associated light chain) at the IgG4 upper hinge (CPSC sequence; less stable disulfide than IgG1/2/3) and recombination with half-molecules from other IgG4 antibodies; DuoBody ENGINEERING EXPLOITS AND CONTROLS THIS PROCESS with asymmetric CH3 mutations:

Phe405Leu MUTATION (on anti-GPRC5D parental IgG4): Phe405 at the CH3-CH3 interface forms a hydrophobic contact with the opposing heavy chain; Phe405→Leu reduces contact surface area → anti-GPRC5D-Phe405Leu IgG4 preferentially undergoes CFE rather than re-forming homodimer. Lys409Arg MUTATION (on anti-CD3ε parental IgG4): Lys409 at the CH3-CH3 interface forms an ionic interaction; Lys409→Arg creates steric/ionic incompatibility for Arg409-Arg409 homodimerization but is accommodated in the Leu405/Arg409 HETERODIMER. The Leu405/Arg409 interface is THERMODYNAMICALLY PREFERRED over Leu405/Leu405 or Arg409/Arg409 interfaces under 2-MEA mild reducing conditions. NET RESULT: co-incubation of anti-GPRC5D-Phe405Leu IgG4 + anti-CD3ε-Lys409Arg IgG4 with 2-MEA 75 mM at 37°C pH 7.4 for 5 h → ~50% conversion to bispecific anti-GPRC5D/anti-CD3ε heterodimer; purification by protein A + IEX polishing yields >95% bispecific purity. Ser228Pro HINGE STABILIZATION: Ser228→Pro in both parental IgG4s stabilizes the inter-chain disulfide → prevents uncontrolled in vivo spontaneous Fab-arm exchange of the administered bispecific product (pharmacokinetic integrity in patients).

SC FORMULATION WITH rHuPH20 HYALURONIDASE: talquetamab is co-formulated with recombinant human hyaluronidase (rHuPH20; ENHANZE technology; Halozyme Therapeutics; 35 kDa; degrades hyaluronan [HA] in SC extracellular matrix) to reduce resistance to SC injection of large volumes. Mechanism: HA in SC ECM (hyaluronic acid glycosaminoglycan; cross-linked mesh; barrier to SC fluid dispersion >1–2 mL) is cleaved by rHuPH20 → transient local HA depolymerization → SC volumes up to ~10–15 mL tolerated. rHuPH20 ALLERGY RISK: bee venom hyaluronidase cross-reactivity (~0.1–1% develop anti-rHuPH20 antibodies; rarely anaphylaxis); VelocityEHS null for talquetamab CAS 2278847-52-6 fails to document this formulation allergen risk.

COMPARISON WITH TECLISTAMAB DuoBody (BCMA×CD3; class 66): both talquetamab and teclistamab use the IDENTICAL Genmab DuoBody IgG4 platform (Phe405Leu + Lys409Arg + Ser228Pro; SC rHuPH20 formulation; Q2W SC maintenance). The DISTINCTION is the first arm: teclistamab anti-BCMA Fab arm (TNFRSF17; plasma cell-restricted; no skin/nail/taste expression) vs talquetamab anti-GPRC5D Fab arm (orphan GPCR; plasma cell-high + skin/nail/taste expression). This TARGET DIFFERENCE produces radically different clinical toxicity profiles despite identical bispecific antibody engineering platforms.

Multiple Myeloma: Pathobiology, Treatment Landscape, and the Triple-Class Refractory Unmet Need

MULTIPLE MYELOMA PATHOBIOLOGY: MM is the second most common hematologic malignancy (~35,000 US cases/year; ~176,000 globally; median age 70); MM cells are malignant plasma cells arising in the bone marrow.
